Appendix 1 WRITTEN APPEAL FORM

Please be informed that, in accordance with the Instruction on organizing the review of citizens' appeals, conducting personal reception, and ensuring access to public information at the National Bank of Ukraine, approved by Resolution of the Board of the National Bank of Ukraine dated May 14, 2020 No. 332-rsh, the client has the right to apply to the National Bank to resolve the issues raised in the appeal, including cases where the financial company did not provide a response within the period established by the legislation of Ukraine for considering appeals or the received response did not satisfy the client.

Submitting an appeal to the Financial Company or to the National Bank does not deprive the client of the right to apply to the court in accordance with the requirements of Ukrainian legislation for the protection of their rights and interests.

Officials of the Financial Company are liable for refusal to accept and review an appeal, violation of the appeal review period, and other violations of the client's right to appeal in accordance with the legislation of Ukraine.

Out-of-court review of consumer complaints is carried out by the Financial Company through the review of written appeals in accordance with the requirements of:

  • The Law of Ukraine "On Citizens' Appeals";
  • The Instruction on record management for citizens' appeals in state authorities and local self-government bodies, citizens' associations, enterprises, institutions, organizations regardless of ownership, and mass media, approved by Resolution of the Cabinet of Ministers of Ukraine dated April 14, 1997 No. 348;
  • The Instruction on organizing the review of citizens' appeals, conducting personal reception, and ensuring access to public information at the National Bank of Ukraine, approved by Resolution of the Board of the National Bank of Ukraine dated May 14, 2020 No. 332-rsh: zakon.rada.gov.ua
